Could you make it a "fillable" adobe pdf file??? You might get more reponses if all people need to do is download the file - type in the anwers - and email it to you. Just a suggestion. Good luck.

I guess you need a real version of one of the adobe products. Or you could just upload the .doc or txt file to make it easier to fill in.

That would work - then either the taker could reupload or give a junk email address you don't care about and they can email the completed form to you. (junk so its not spammed to death if you use it reguarly).

Same as a picture file you upload:
Just make sure the file name isn't capitalized or has any spaces in the filename - for some reason php doesn't like those.

This whole questionnaire gives me a fishy feeling. What is it for?
Also, the layout of the questions is inconsistent. For example, the first question asks if the government has the right to view IM, but then asks for a quantitative response (i.e. a scale), then later on the last question on page one, it again asks if the government has the right to watch comp files w/o a warrant but instead wants a qualitative response (yes or no).
Then on question 5 for public surveillance it is essentially asking if spending should be increased for public surveillance to improve security. That is loaded. What if you answered the previous question (#4) with "Not at all". i agree that many of the questions seem ameteur, but i should have revealed the whole reason why i posted this survey. my son has an assignment due in class, to get 30 surveys filled out. the questions were created by the students. they are studying how surveys affect governmental decisions. i understand that the questions could be better, but i dont believe the students understood how they wanted to ask the Q's, so please bear with it thanks.
