I've tried every size and brand of case on the market and there has never been any reason to adjust for anything other than the spine width. Pretty cool huh?
I have had to adjust and re-adjust, and I have bought all of my cases from either Meritline or Supermediastore, and all of them the same two lines. I have had to us:

7.23 " by 5.85" (these were the earliest ones, a 200 pack, the ones that had the actual DVD logo on the inside of the case)
7.23 " by 5.82" (these were also ones with the imprinted DVD logo on the inside of the case) They were also made out of a superior plastic to all of the others -- nice and sturdy, even though they were purchased from the same source as the much flimsier ones below, and were supposedly the same "line" or "model" as the ones which follow.
7.22" by 5.81 (these settings were fine for quite awhile, up until about eight months ago)
7.22 by 5.78 (my current settings. The glue used to bond the clear plastic on the outside has clearly been moving further and further away from the outer edges, as a trend. This is true on tops and bottoms, and sides, both on the front and back. Thus, my paper covers have had to get progressively smaller.)

As LordSmurf correctly stated, the variance is most often due to the bonding of the clear plastic, and exactly how close it is to the outer edges. I have had to change settings even within the same shipment, twice, when one box of 100 was (obviously) different from another. Upon cracking open the new box, every cover suddenly could not fit properly without a severe "curling" and "warping."

You do not have to like his response, but it is most certainly an accurate one, which is not at all true of the statements you posted.
