For example, combining the main movie, the outtakes feature, and the "making of" feature into a single seamless output file that plays all three beginning to end. MP4/MKV desired but Xvid is ok too.
I have tried the recommended programs "mkvmerge" and "DVD2one" but the documentation just doesn't cover what I want to do. Is there a cookbook guide somewhere?

Sequentially rename all of the VOB files and join them together with something like VOBMerge of VOB2MPEG first....then do your conversion.
 Or this method:

	Without a menu, try dvdshrink

(Hint: Leave the compression set to 100%)

 With a menu, try avstodvd

My test DVD has five titles, the two I want to merge are 1h34m and 14m long respectively.

I tried VOBmerge, combined all the vob's in the VIDEO_TS to one big file. The resulting "merged.vob" was only 39 minutes long when played in VLC.

Haven't tried the vobedit/ifoedit method yet as I have about 120 DVD's I want to process and I was hoping for a solution with fewer steps.

The programs you suggested seem to be for combining several titles onto a DVD (or DVD image), but they remain separate titles. What I'm trying to to is select some of the available titles from a DVD, and then rip those titles to a single, merged output file, rather than individual output files for each title.

 thanks anyway

Check the properties of each title. For example, if the main movie is 16 X 9 and the other title is 4 X 3, strange things can happen. Main movie may have multiple audio tracks and the others only one, etc.

I use Videoredo when I want to join vobs. It will give an error message if the properties of the vobs don't match.
