  2. I know CDex can list everything(artist, album, etc), you can even change the separator.

I wouldn't worry so much about the TAG format, choosing the right software and encoder is more important

I use dBpoweramp and it lets you choose the TAG output directory name, I usually go with "01 Artist - Song"

EAC (Exact Audio Copy) also allows for you to set the tags and output name in anyway you want and it automatically retrieves the cd info from an online database.
It will even test all your drives if you have more than one and tell you which one is better for the most accurate ripping.
It works with a technology, which reads audio CDs almost perfectly. If there are any errors that can’t be corrected, it will tell you on which time position the (possible) distortion occurred, so you could easily control it with e.g. the media player. With other audio grabbers you usually need to listen to every grabbed wave because they only do jitter correction. Scratched CDs read on CD-ROM drives often produce distortions. But listening to every extracted audio track is a waste of time. Exact Audio Copy conquer these problems by making use of several technologies like multi-reading with verify and AccurateRip.
I used to use CDex many moons ago and loved it but switched over to EAC quite some years ago and never looked back.

You will not beat it for features or quality.

Plus, if your songs' tags are in order, you can always use a tag editor like mp3tag to change/restructure the filenames to the way you mentioned, above (or any other way) easily.
If cameras add ten pounds, why would people want to eat them?

  6. CDex does it nicely, if tag is configured properly and all information is provided like Album, Artist, Music Type, Year and Rename the tracks to the real names. Otherwise mp3tag will do after ripping.
