A quick search on Amazon for "dvd inserts" should get you a list. I've found it cheaper and much better quality to use 8.5x14 paper and cut them myself. I am currently using this paper: http://www.newegg.com/Product/Product.asp?Item=N82E16828103049

You can buy these at any of the following stores:
Best Buy; Future Shop; Staples; Business depot and Grand & Toy.
The first two sell the brand names - Neato, CD Stomper etc. The other three also have their own make. Check them out online.
I do the same as Krispy Kritter and use good quality paper and cut them out. -
What a coincidence, I just got another batch this week. I get them here, along with other DVD media supplies regularly - good prices and reliable service.
