The problem seems to be that I'm only wanting to convert "Title 4" of the DVD according to WiMP. When I open the IFO file AutoGK asks me what I want to extract and I select "PCG 4".
The sync issue that I'm having is because AutoGK starts encoding the video from "Title 1" (the FBI and copyrite warning) and then continues through "Title 2", "Title 3", and finally to "Title 4". The audio seems to begin immediately from "Title 3". In case the "Title x" is confusing that is what I'm getting from WiMP on the right side...."Title 4" has multiple chapters inside it.
Changing the MP3 from VBR to CBR makes no difference in the result.
I've tried this with Fair Play as well and get the exact same result.
Any suggestions as to how I can make AutoGK, FairPlay, or some other tool actually start at the right place?

Decrypt using DVDDecrypter set up for IFO Mode (Up at the top, Mode->IFO), and choose the PGC you want. It's going to show you the longest PGC. If that's not what you want, look around for the PGC you do want. Then feed that into AutoGK.
Another way, if the vobs are already on the hard drive, is to use PGCDemux with "Create a PGC Vob" box checked. Then choose the correct IFO, and in the dropdown box, the correct PGC of that IFO/VTS. -
