  2. This may be a stupid question but have you enabled tv out in Control Panel?
What drivers are you using?I recommend Cat. 4.12
Do you get an exclamation point(problem) in Device Manager?

You also have to reboot after you enable TV out.

  5. IMHO(and most folks at www.rage3d.com) any Cat. newer than 4.12 is not needed for older AGP videocards.
Here's how I install new drivers:
    1.Uninstall ATI MMC,don't reboot.
    2.Uninstall ATI Control Panel(or CCC),don't reboot.
    3.Uninstall ATI Display Driver,reboot.
4.Windows will detect new hardware, hit cancel,go to regedit, LOCAL_MACHINE,Software and delete all ATI folders, reboot.
    5.Install ATI Display Driver,reboot.
    6.Install ATI Control Panel(or CCC),reboot.
    7.Install ATI MMC.
8.Right-click on desktop,Properties,Settings,Advanced,Displays and set max. refreshrate and enable tv out.
