inserting extra vobs

hope someone can help,

i have done many backup and know how to do movie only rips as well by using dvdshrink/dvd2one. What i would like to do now is to insert an extra vob before the movie starts. The VOB is a dolby digital/dts test video downloaded from this site http://www.cinenow.com/us/vobtrailer.php3

if so would i lose the original chapter points when creating new ifos & bups,
