Check the guides to the left.
Basically, you will need to rip the subtitle stream (Subrip or Vobsub), then use Virtualdub with a subtitle filter to add the subtitles to your D2V file (either convert it to a "fake" AVI directly with VFAPI, or via TMPGEnc with some manipulation) - then frameserve from Virtualdub back to TMPGEnc for your final mpeg file. Don't try to incorporate the audio until the last step would be my advice, otherwise you could run into synch problems.
