    Is it possible to do this?

    For example...

    1. Have movie in quicktime format.
    2. Plays from website.
    3. Give user a form at the end of the video.
    4. Capture data
    5. Then encode video with data that was given at the end of the video.
    6. Provide download when finished.

    you mean the user fills out a form in which the forms data is added to the video?

    This could probably be done much easier in Flash where the form's data is sent to YOU via email. Then you simply click a few buttons, add the form's data, join it to the video, then upload it. With the help of a programmed cursor, it would be mostly automatic.
