Step by step guides on authoring can be found at VCDHelp's How to author.

You definitely should have a look at "How to create a VCD or SVCD menu and chapters with TSCV" (on the aforementioned page).

Regarding still mpegs's needed to create the menu's:
Be sure to drop by at www.geocities.com/mikk999/VCDStills.htm (mentioned by Vitualis, not sure). This method has worked for me. Using the menugenerator in TSCV screwed up my menus.

I know it's alot of info but I managed to grasp the basics in 2 days or so. It's rewarding though.

  3. You cannot use Nero to make true chapters. It can make a multitrack VCD with a menu (but this implementation is somewhat limited and buggy).

VCDImager with one of the GUIs (e.g., TSCV or VCDEasy) is a good start.

If you are interested with how things really work (the guts behind the authoring) or are interested in authoring the VCD manually (which is the most flexible and powerful way of authoring), then I suggest reading some of the articles I have written here: http://www.geocities.com/medinotes/vcd
