When I started middle school (junior high), and showering after phys. ed. became standard, like everyone else I checked out how everyone else was equipped. I seem to remember only one or two boys who hadn't been "cut". It's just assumed it'll be done unless the parents object.

My Wife's sisters sons are all "anteaters", however, for religious reasons.

When I shower my feet are in the water. That's good enough for me. When I shampoo my hair, soap makes it to my toes during the rinse. If I feel they need a good cleaning, I stomp in the water.
