I simply create a box in MS excel, adjust it after a few tests prints (just the box/border, draft quality) and then use 'insert picture'.
Perfect size every time (until I swicthed suppliers and the blummin' cases are just a little different to the last batch!)

Scan them myself, or if I dont like the original I'll look on DVDcoverart.com for a better one.
I print them with CoverXP, using Meritlines glossy A4 sized DVD Covers. Works great. -
I hardly ever use full DVD cases for backups, as I do not like the amount of wasted space they take up. Standard CD cases work great for me, and there are tons of tools that do covers and cases for that size box.
The only plus that I find with full size DVD cases is with the type of plastic used. The jewel case plastic can be annoying because of the ease of breaking compared to the DVD box.
