AviDemux2 will convert an Mpeg2 program stream file to transport stream without re-encoding. The resulting file works with HD hardware decoder cards (ie - MYHD MDP-120).
To be 100% certain it will play on other hardware players, run the resulting .TS file through Mpeg2Repair. That will move the PIDs to 11 and 14, quite standard for most video cards.

hi, I tried this... but run into the following issues:
1) When opening the mkv (video:h246 /audio:ac3) first I receive the message:
H264detect: If the file is using bframe as reference, it can lead into crash or stutteting. You can user another mode.... doesn't matter what i choose, I see the video...
2) then, when just clicking on the save button, it Sais, "This is not MPEG compatible, you can't use copy codec". but as far as i know h264 and ac3 should in a mts/m2ts container (http://en.wikipedia.org/wiki/AVCHD) or not?

Should have mentioned, this is probably for Mpeg2 media only. H264 can be done from the command line, theoretically at least with the combination of several tools.

Yes, h264 and ac3 should mux into a TS file, but the problem with the command line software I suggested is that it will convert from MP4 to TS, and MP4 will NOT support AC3
Painful, huh ?
