I have attached a pic. Is this due to dirty heads or just a bad tape?

Clean the heads and then you can rule them out (or in). Could be a disintegrating tape or the recording alignment disagrees with your player (common problem with LP tapes).

They are so hard to access. I had to use a pole to clean the heads.
But it seems to solve the problem even though there were hardly any dirt on the Q-tip. -
Next time try an electric toothbrush and some Comet.
Seriously, this is one reason to fast-wind a cassette end-to-end before you play it. It encourages the loose junk to fall out so it won't clog the heads. -
Same problem for me with LP miniDV tapes. I cleaned the head with cleaner cassette and rewinded the tapes couples times. No improvement. Same camcorder and same tape. Older SP tapes plays without glitches.
