    Assuming that you have one, I find that the best way is to simply scan an exisiting cover (and then make a mask -- especially for irregularly shaped covers/inlays like for MiniDisc) or for DVD/CD covers, just get out your ruler and make precise measurements and make a template using Paint Shop Pro or Photoshop.
