    There's really no telling what software was used, as far as I know, I would try to contact the person on Youtube who uploaded it.
    It's not important the problem be solved, only that the blame for the mistake is assigned correctly

    I see 4 transitions:
    A blur-3dviewport move-zoom+unblur
    A whiteout-blur-extrude/verticalstreak-push-unblur
    A cut to faux bad tv/vhs signal cut to next
    A feax matrix/green crt cut/push/wipe to next

    With the exception of the 3d viewport move (which ought to be done by the same app rendering the 3d game), all that stuff can either be gotten from online or created from scratch and executed/rendered in any decent NLE.
