Can someone tell me whether an interlaced video file is caused from a bad encode or a codec issue??

Neither bad encode, nor codec issue. Interlaced video is the native state of NTSC and PAL broadcast (TV displays only interlaced still, progressive TV are very very rare if they exist at all) and of NTSC DVDs which are telecined (usualy in case of Holywood movies) or true interlaced (in case of some shows) or the worst scenario - mix of telecined content and pure interlacing - classical example are StarTrek (don't know which exactly it seems for season to season they are different!).
