I already got something that scans and gets rid of AdWare. I tried Spy Killer for the hell of it and supposedly I had 72 infected things. Most were "registry keys". My other spyware scanner never picked up on these things, although I have gotten rid of a lot with it. But 72 files it missed?
Is Spy Killer worth $40? What could happen if I do not get rid of infected registry keys and things like that?

I doubt if it's worth it. Ad Aware or what I use, Spybot work fine and they are free. Most of your 'spys' are cookies from advertising companies. Only a few are malicious and you usually get them from places on the net that you shouldn't go to anyway.

SpyBot is fast, works great and it free.
