Is there any option or effect in Premiere 6.5 that I can fade in/out video clip from/to black?

You need to use a transition. File->new on the menus gives you the option to create a black matte video. Drag this to the time line and use a "cross fade" transition to go from/to your video.

RUSSWCJ is right, use a cross dissolve. Drag the transition item to the transition clip towards the end of the video clip. Make sure you choose the right fade direction, from A->B or B->A depending on where you put your video clip. (An empty clip represents a black screen, you don't need to insert a all black photo).
Extra: If you want to fade an title say from a psd file(usually transparency set to Alpha Channel), you can fade the title by selecting the video gain level by clicking on the > black arrow on the left most section of the video channel. to make the arrow point downwards.
