I was playing around with one pass vbr in CCE and got results equivilent to 1 pass +vaf (which takes 2wice the time). I was just wondering how there can be a difference in singal pass and multipass if they are both spreading the bits according to scene change. I mean if 4 pass + vaf is just working out when to up or lower the bitrate then surely singal pass with a quant of 7 should look just as good as it still works out when to lower or higher the bitrate!

Or is there a difference in how the bits are spread out accross the picture?

Each time that CCE passes through the video file, it updates (or "refines") the .VAF file. After about three passes, the refinements get harder and harder to detect, so diminishing returns occur for any more passes. This is true for normal bitrate DVDs (4 to 5 Mbps) - for files based on borderline bitrates, like SVCDs, diminishing returns may not occur until 5 or 6 passes.
