Alright, so I made an account to ask this question, as I am pulling my hair out at this issue I'm having.
I used MKVExtractGUI-2.2.2.8 to unpack an MKV file. The two files extracted read as follows:
Persona 4 The Animation Ep.01 - You're myself, I'm yourself_track1_und.h264
Persona 4 The Animation Ep.01 - You're myself, I'm yourself_track2_eng.aac
Using YAMB, I muxed the files together and I proceeded to do this for the entirety of the show. However, when I played back the mp4 file, only the audio would work. I was presented with naught but a black screen and some crisp audio.
I tried uploading it into Sony Vegas (which is why I am dong all of this), and there is no video there, either.

Try mymp4boxgui also. I think yamb is using an older version of mp4box.
Or try mkv2mp4. -
Just tried and no luck. I'm still having the same exact problem.
I should also mention that I have other mkv files that I have successfully converted without issue (except a little audio delay). I am unsure of what the problem could be, but the entire batch of episodes doesn't work.
EDIT: It's also worth adding that the file works just fine in VLC Player -
POst the details from the mkv. Use media info, view->text.
