Hi,
I have 2 m4v files, A and B. I need to insert B in the middle of A. A is about 90 min long and B is about 30 second. Is there a tool that do this?

Maybe avidemux.
Open A m4v, set start[A-button] at the middle and stop[B-button] at the end of the clip.
Press the delete-key to remove it.
File->append B m4v.
File->append A m4v again.
Set start and stop and delete the other half.
Save as new mp4.
You could also try videoredo tvsuite h264 or solveigmm video splitter. They are not free though.
