Looking for an implementation of the FFMEPGX app to take a Flash input stream and convert to H.264 in near real time.
Something like: Read in a Stream ID from and XML file in a monitored directory (or periodic database query), begin converting into stream that can be accessed via temp URL until the input stops. This would either be a modification of the basic software or instructions of how to do it with what I have now.
Don't expect it for free of course.

I really don't have the technical skills to help you out, but feel free to get some ideas from this Loop Transcoder script I threw together. I set it up to wait for .flv files to process.
Run Loop_Transcoder.bat, then drag an flv file into the same folder. It'll find the file then transcode it to h264 MP4

More professional would be to use something like Procoder or similar, which uses a Watch Directory to automatically process into your desired format.
