?New? Looping Issue with Backups of Live Free Die Hard and Deja Vu

I used the search and couldn't find anything similar, but please link me if this issue has been discussed.

I have recently ran into a few discs (Deja Vu, Live Free or Die Hard) that backup without error but when you go to play them the movie will loop 5-15 second sections of the movie 3 times and then continue on with the movie until doing the same thing again minutes later. I have this issue with the DVR on both my Panasonic DVD Player and the VOB files on my HDD. The movie time and chapter points seem to get really messed up during these loops, and I can't quite ID the pattern...it will jump forward 2 min, then back 2min and change chapters, but the behavoir on the movie is as described above.

I use DVDFab and DVD2One with Recordnow Max to burn on Taiyo Yuden 16x DVD-R's.
