My 19" Samsung widescreen didn't hurt my pocketbook at all! $129 at Staples last Black Friday sale.
Now the dual 22" widescreens they just put on my desk yesterday....
Those hurt my neck from the constant shifting back and forth to take it all in! -
The dell 22" is cheap .it's only $296

For refresh rate, anything above 60Hz is good. (Probably >50Hz in PAL land.) Though it doesn't seem to make much difference with LCD monitors. I know a CRT monitor is tiring for me to look at if it's set a 60Hz, especially if you have florescent lighting. Just changing them to 75Hz seems to help a lot.
If you are concerned about long working times in front of a computer, you might look here for some useful suggestions: http://www.cdc.gov/od/ohs/Ergonomics/compergo.htm
