    I appologize if this is in the wrong forum.

    I am planning to shoot a music video for a band. Everything is in the thinking stages right now and I need to know where I can get some release forms as far as locations, extras, and what ever else i will need release forms for. I appreciate a point in the right direction. Thanks.

    You can Google on "video release form" and get plenty of hits.

    The Video Development Initiative Workshop has some sample release forms you can customize for your particular needs.
