Fuji 8x +/-R 25-spindle for $9.99 + $5 rebate @ FYE

I rarely go to FYE stores, but I happened to catch this today. They have Fuji DVD+/-R 8x 25-spindles on sale for $9.99. With a $5 mail-in rebate good till 3/15 cuts price to $5 for 25 DVD's ($.20 each). Most of them I saw were made in Taiwan, but I did spot a couple +R spindles made in Japan which are TY discs. Rebate form says "limit 2 rebate checks per item". They also have $10 rebates on Fuji 30- or 50-spindle CD-R media (again, some made in Japan, some Taiwan).

I was at the mall and remembered this, I saw some packs of Fuji dvd+r Japan for 9.99, but no rebate form. Does it print out when you buy it? Or where would I find the rebate form?

I had to mention it to the cashier to get the rebate form. I guess they keep them behind the counter. But once I mentioned it, I got the rebate form and a duplicate receipt.
