Looking for a few templates

I'm compiling a DVD which includes a music video with a strobe light through the whole thing as a special feature, and it's a bit painful to watch if you're not suffering from epilepsy, but if you are, chances are you'll get a seizure.

For this reason I want to include a warning before this gets played, and I'm not sure what to write on it. I'm looking for some template warnings including copyright information and others you might find on a DVD. IF anyone knows for some websites or has some templates, please share, any help is appreciated.

I searched Google, but couldn't find anything that was any use to me.
