Check out DVD-Lab (in the Tools section at left) a quick read of the 3 page "Getting Started" file and you should be able to create menus and chapters easily.

If you're looking to add a menu to an existing dvd, I don't know any way to do that - you'd have to rip and reauthor the disc.

I keep forgetting that the majority here are into ripping off Hollywood DVDs.

I am into creating custom DVDs from video or photo/slide shows. That is what my company does, if I want to watch a movie, I will rent or buy it.
