unless im doing something drastically wrong, i cannot find anyhwere in neros cover desginer on how to make the image vertical (or in other words how to rotate it) for one so it fits on one sheet of paper and two so i can write on the spine

What version you got, I just loaded Nero DVD and it has the following;

Booklet
Insert
Disk

They all print on single sheets, also right click > effects > rotate

im using nero 5590 and that choice you mentioned is defiently not there

the dvd case temp is in 5.5.10 if you wanted to know i just updated my nero.
