Printing DVD cover template

What is this type of DVD/CD case called?


I'm trying to find a template so I can print covers for that format(back, front and sideways), anyone have any idea where I can find them?

It's a jewel case.

I normally use SureThing or RonyaSoft for printing covers.
There are probably some good freeware alternatives though that someone can recommend.

First and only time I stuck a label on a some CDs it made a few of them unreadable

None of my CD players can read any of those covers or cases either, label or no label
