I have found with many DVD/.vob files that ffmpeg fails to recognize that these are NTSC Film sources using 3:2 pulldown, and therefore cannot handle the sync of audio and video, or otherwise properly process the file.
I use, variously, OSEX, Extractor, yade X, DVDBackup to extract single VOB files from DVD and sometimes the file is read correctly by ffmpeg, sometimes not.
Usually, the file is identified as 29.97 fps (NTSC Video) -- sometimes it is correctly identified as 23.976 using 3:2 -- the results are almost random.
If I use mplayer for decoding, it will often correct the error when it recognizes the 3:2 source, however, some files appear to flip back and forth.
Is there any way to fix this? I have tried setting "Set 3:2" and setting the frame rate to 23.976 -- this results in jerky video which rarely syncs with audio.

I have also noticed that the 3:2 pull down seems to go on and off while converting, ofter resulting in non-synced audio. If you use Quicktime to decode it will usually decode it right and the audio will end up in sync at the end. give it a shot. This will only work with a VOB and not a VIDEO_TS folder though. Use 0SEx to convert the main feature into 1 large VOB and everything should sail smoothly.
