1. Capture a DVD in AVI uncompressed
2. Resolution 320x240
3. Allow me to capture with an animated logo. I don't want to add it after the file is captured. I want the logo to be added while it is capturing.
4. I'm looking for a card that uses codecs that are compatible with Quicktime.
Any card can do the above?

actually my osprey card can do most of these things...
#1 and #2 most card can do it of course
#3 osprey can do it but the logo isn't animated
#4 if I capture my avi with osprey I can open/edit them in quicktime no problem as it uses codecs supported by quicktime -
I'm not sure I understand a combination of capturing uncompressed avi AND using a codec at the same time.
However, I believe you could capture with Virtual dub and add the logo filter. With this you could add a animated logo as you capture. Given the small frame size, I'd guess processing would not be a problem in real time. Here is a link about the filter.
