It seems that it never happened to me... but now ripping "Sum of all fears" I get a great problem with audio. Around the second half of film my audio goes out of sync... I don't know why. I have extracted elementary streams with 0sex, reencoded mpeg2 video using ffmpeg 0.0.6g and then muxed together with DVD SP. I always used the same proceeding but I have never had sync problems... what's happening?

There can be many reasons for this. I have often experienced synch problems with films that have frame rate changes partway through. Usually, though, frame rate issues are at the beginning of the movie.
It might be that your particular disk has a troublesome layer-change spot. Remember the warnings on the cover: "might trigger a slight pause."
It's possible that when ripping it hit a bad spot and mucked up the video.
Try ripping just the chapter where the movie goes off sync, and see what results you get.
