I encountered similar issues, which I am still somewhat struggling with.
I ended up with 4 CD's (meaning: 4 MPEG2 files), which I could not join in sync.
Finally, I de-multiplexed all 4 CD's into video and audio, using TMPGenc.
Then, I joined all 4 Video files, joined all 4 audio files, then re-multiplexed.
the result is a single MPEG2, with everything in sync.
Now, for some reason, SpruceUp will not recognize the file, which means I have not yet been able to export the MPEG to a DVD-recognizable file.

If I can get my hands on a competing conversion tool (Ulead DVD workshop or something), I wil try that. In the meantime, I have run the combined MPEG through DVD2AVI again, to create a .d2v file, and I'm trying different re-encoding options in tmpgenc, using the .d2v and the mp2 audio file from the demuxing.
