    What is this kind of video artifact called? And how is it most effectively fixed?

    Triple ghosting. Probably unfixable if you don't have "original" video.
    Anyway, you need to share a video sample if you want the better answer.

    And how is it most effectively fixed?
    if the original frames/fields that got blended together are also part of the video: removing the blended frames for example with sRestore (Avisynth/Vapoursynth) otherwise I would recommend trying ExBlend (Avisynth: https://forum.doom9.org/showthread.php?t=175948) or best deleting the file.

    Maybe that ghosting is an intentional special effect done by a prominent director.
