I have a strong memory from within the last year or so of using some Windows-based tool that worked very well in achieving what I described in the OP's title. But for the life of me, I can't recall it's name! It seems that none of the tools in this Subtitle Editors/Converters area work with those file types. All those that I tried seem to require avi or DVD/BluRay folders or sub files. I can't even recall if it was free/open source or commercial.
Does anyone know if such a tool exists and where I might find it?

I guess VideoSubFinder.
You can try a tool in my signature if you know how to use Avisynth.
