My guess is you made the AVI file using the Bob Doubler filter in VirtualDub. Then opened the AVI using that same instance of VirtualDub. The program doesn't clear out all the filters when you open a new file -- meaning the Bob Doubler filter was still active. So you were bob doubling twice, hence the 4:6 repeat pattern.
No, if the percentage is anything less than 100 it's a mix of hard and soft telecine. For example, 99 percent soft telecine, 1 percent hard telecine. But when there is only a little hard telecine you can often get away with ignoring it, treating the whole video as soft telecine, without getting audio/video sync problems, or being left with a lot of interlace comb artifacts.
