  1. What is the cause of this problem? I had this problem before and it was solved simply by reinserting the tape. I have fast forward and then rewind the tape but it didn't solve the problem.

    Looks like the tape has be scratched. I know I've seen that on a standard VHS which was scratched the entire length of the tape by a piece of crud stuck on the guide posts.

    could be a dirty video head. i'd try a head cleaning tape.
