What exactly is a single inserted frame? What does it mean for the authoring process? Are they easily identifiable after capture and is it a sync issue that needs to be fixed after capture?

When you capture at a fixed bitrate but the capture device isn't giving you frames fast enough, the capture program will insert frames to make up for the difference. The inserted frames are usually copies of the last frame received.
