  1. I have two audio files (AC3@192kbit/s) I would like to combine into one AC3. The volume on the second audio file is way higher than the first one. Is there a program that lets me reduce the volume on the second audio file to match the first one without too much quality loss?

    Sure, you can use the free Audacity for that.
    Just raise the volume on the clip with the lower volume but make sure you do not clip (e.g. go into the red).

  3. Check metadata - i think that Dialogue normalization value can be different for both files (for example 0 and 31) - maybe you can set 31 on both?
