  2. Delete the index file and try again.

    Code:
    D:\Videos\Anime\[HorribleSubs] DanMachi [1080p]\[HorribleSubs] DanMachi - 03 [1080p].mkv
is the file FFVideoSource shall load.

    Code:
    C:\Users\Jeroen\Desktop\Vafe 13 (2013.07.18.06)\indexcaches\b58f1fb1a04ad36c52e2d8d426ccb8c8\b58f1fb1a04ad36c52e2d8d426ccb8c8.ffindex
will be the matching index file. Delete it. Maybe by pressing the "Del" key on your keyboard when this file is selected in your Windows Explorer.

If this index name is generated randomly on each run of this software, it may be hard to track down anyway... possible that my suggestion already came too late, the next run may have a different result.

  5. Grrr, what a worm of a file name. This board software keeps inserting spaces on its own.
    Hint: use 'code'-tags

    Thanks!

  7. Some programs have problems with particular characters in file names. Try renaming the files without square brackets. Or any other non-alphanumeric or foreign characters.

@ jagabo: The error is related to the index file, not the media file. And the index file name is completely alphanumerical.

Just the location is nonsense (on the desktop); but well, IMHO.
