In the past I used Gordian Knot or AutoGK to back up my personal dvd's into xvid or divx. With good results!
My goal is to produce very nice quality videos using recent codecs / formats / containers, just like : H264, H265, MP4, MKV, x264, x265, etc.
1) Do you point any specifical codecs or containers to achieve nice quality ( audio / video ) ?
2) Which programs do you recommend to produce high quality output material?
3) Any portable program available to do the job?

I used AutoGK for quite a while myself. When I moved to x264 encoding I switched to MeGUI. It's not "auto" like AutoGK, but it uses many of the same tools to do the work (Avisynth, DGIndex, etc for DVD encoding) so it should be a little familiar in that respect.
You can enable x265 encoding in MeGUI's options and it'll download the encoder for you when it updates, but I'm not sure x265 encoding is worth it yet. It's still early days for the encoder and very little hardware supports h265 playback.
MeGUI is portable. You just download it, unzip it, and run it. No installation required. It also includes a portable version of Avisynth, although it'll use an "installed" version if you tell it to. I think the only requirements are the Haali Media splitter (it needs it for the decoding of some file types) and a minimum of dot net framework 2.
