Try using a program such a VirtualDub to resave the AVI without audio. Just copy the video.
Even though everything looks okay I've had a similar thing happen a couple of times (I don't use Premier) and removing the audio fixed the playback speed which led me to conclude (guess) the player isn't getting the audio sample rate correct. I think when it happened to me I re-encoded the audio and added the new version to the video which fixed it.... maybe.
It's at least something to try.
