need to add a static graphic and title in Premiere

I've done a short video. At the end, I want to add seven seconds of black, fade up a jpg and a one-line title beneath it. I'm sure there's a simple way to do this but every time I look at the documentation, my eyes begin to bleed. Please somebody help me.
I'm on Premiere Elements 9.
