"No difference at all." -> since I neither got iTunes installed (even not on my mac) and I don't own an Apple TV I can't say where to look, but if changing the font Size from e.g. 18 to 24 doesn't change anything, the ttxt styling clearly gets ignored and there either is a setting in either iTunes or the Apple TV to change this or this is some sort of bug or missing feature on their end.
-> Sorry, that I can't help you any further, but this seems to be a problem where somebody with iTunes&Co is needed.
(I know alternative software to do the job, but that won't help when the styling is ignored.)

	I guess: 
 QTText should be possible if your output container is .mov, but I don't know if there's a tool out there that can convert srt to QtText (if you are a programmer it shouldn't be hard to write such an application).
