Printable DVD Slipcover Templates

I had a good look around Google for printable DVD slipcover templates, like something that would be found releases Resident Evil Degeneration, something simple that could be printed on my Epson R220, I've looked around and can't find any kind of blank DVD slipcover template for customization, would any one here know of any for download! or maybe a paid template, thanks.
