I am using Tmpgenc xpress 4.0.

I would like to know Image Resolution & aspect ratio value.

    MediaInfo 0.4.0.1:
    codec: DivX 5
    W640 x H480
    Aspect Ratio: 4/3
    Frame rate: 29.97

Should I set it NTSC? Is it depend on source file or TV?
Drop down menu don't have 640x480. What should I set? Should I set it highest or customize it to 640x480?
There are many 4:3 inside aspect ratio like pixel 10:11 (NTSC4:3) , display 4:3 and so on. What should I set?

Yes, NTSC. Yes, NTSC 4:3 (720 x 480 is the resolution you want). Chances of getting a "High Quality" mpg file are slim to none.

You can feed AVI files directly to TMPGEnc-Author, no need to use Express at all. It will default to the correct settings too, or you can adjust for best quality.
