I am tryung to create a survey with those radio buttons 1) very satisfied 2) satisfied 3) neutral 4) unsatisfied 5) very unsatisifed.
Is there a way to create those radio buttons and have them filled in automatically when users click on it? I cant seem to find them in 2003 Word.

If it is the latter, you go into Tools->Macro->Visual Basic Editor, then click on the Design Mode button (about sixth from the right).

not creating an html file for web page. i guess its a macros? can i do it without using macros?

What exactly is your goal? If you are making a fillable form, try Adobe Acrobat (PDF), which is more portable. Word is too proprietary in that respect (macros & visual basic).
You may also want to try what was suggested: a web page. You can find out how to do what you want here.
Here is an example of what you are trying to do.
